Who Are WE?
WHat matters to you?
‘What matters to you?’ is an initiative, developed by Health Information Scotland, encouraging more meaningful communication between people who provide health and social care and the people they support or care for.

This initiative has been adopted by a collaborative of statutory and voluntary End of Life Care organisations from across Dorset who are very keen to work together to help improve the quality of end of life care for everyone; regardless of diagnosis or postcode.
By supporting health and social care professionals to enable them to have more meaningful ‘what matters?’ conversations with patients, they believe this simple approach will ultimately help to improve the quality of life for the individuals and those closest to them.
Ask what matters, listen to what matters and care about what matters.
